If you need to cancel or move your appointment, 24 hours of advanced notice is required. In keeping with our regulatory bylaws, we kindly request that you complete our Communicable Disease Screening Patient Questionnaire prior to your visit. All information provided will remain strictly confidential.
Cancellations and no-shows are subject to the full appointment fee for missed appointments. Please talk your RMT if you have any questions or concerns about canceling or rescheduling your appointment.
